A newly developed service in partnership with the HSE in Cork is hiring Staff Nurses and a Clinical Nurse Manager.
A Transitional Care Unit that is specifically designed to enable patients to step down from acute services to an area of rehabilitation, re- enablement with a focus towards supporting a successful discharge home.

About the Role:

Carrying out a range of duties to ensure that the person centered care and well-being of residents is met satisfactorily, including supervision of all staff.

Qualifications:

Candidates must be registered with An Bord Altranais.
Strong clinical & communication skills are required.
Ability to work as part of team & independently.
Knowledge of HIQA Regulations and standards is essential.
Main Duties & Responsibilities:

Assume responsibility on each shift for the nursing care of allocated residents.
Ensure a high standard in care for the patient is maintained at all times.
Lead the Team of Healthcare Assistants in providing high quality direct and indirect care.
Build strong relationships with patients to encourage trust, and understand their needs.
Ensure Safe medication administration and management procedures
Recognition and appropriate management of a deteriorating resident
Clear communication with patients and all members of the multi-disciplinary team
Ensure that all patient assessments and care plans are completed with a person centered approach to optimize patients journey, ensure all nursing documentation is continuously updated.
Evaluate resident care needs and in-conjunction with our multi-disciplinary team implement and evaluate individualised care plans that meet the resident's needs.
Maintaining accurate documentation relating to residents in accordance with regulatory requirements.
Shifts are rostered Monday to Sunday and will include Night duty.

Registered General Nurse and registered with the NMBI with full work permits
Excellent patient care to a high standard
Excellent interpersonal skills and proven written and verbal communication abilities.
